RAFFLE DRAW MECHANICS

1. Every P20 single receipt purchase of the 2009 Mangaldan Town Fiesta Celebration RAFFLE TICKET entitles the buyer to join the RAFFLE DRAW.
2. Purchase of one booklet (equivalent to 10 Raffle Tickets) entitles the buyer to two (2) free Raffle Tickets.
3. Buyer must fill in the raffle coupon with his/her complete name, address, contact number/s, affix signature and drop at the designated TAMBIOLO in front of the Mangaldan Municipal Building starting January 01, 2009
4. The Grand Draw will be held at the Mangaldan Public Auditorium on March 06, 2009 at 8:PM during the PEOPLE’s NIGHT Program.
5. Prizes are as follows:

1 - 2nd Prize                      :        SAMSUNG 35-INCH LCD TV
1 - 3rd Prize                       :        SONY DIGITAL CAMERA

10 - Consolation Prizes        :        4 GIGABYTE USB

6. The prize is not convertible to cash.
7. The winner will be notified through announcement during the RAFFLE DRAW, registered mail and his/her name will be posted in-front of the Mangaldan Municipal Building.
8. The winner must present the winning raffle stub and valid proof of identification with photo and signature (i.e. driver’s license, passport, NBI clearance, SSS, TIN, etc.) and the registered mail in claiming his/her prize (if not claimed during the RAFFLE DRAW).
9. The winner can claim his/her prize right after the RAFFLE DRAW or at the Mangaldan Municipal Building.
10. In the event, the winner cannot present the raffle stub, he/she needs to execute an affidavit of loss duly notarized. A 60-day waiting period shall be imposed as soon as affidavit is submitted to our office in order to ensure veracity of claim. This is for the protection of the winner.
11. If the winner is a “minor”, he/she must be accompanied by an adult who must, likewise, present his/her own set of identification papers in claiming the prize on his/her behalf.
12. Redemption of prize shall be within sixty (60) days from receipt of notification; otherwise the prize will be forfeited in favor of the 2009 Mangaldan Town Fiesta Executive Committee.
13. All Prizes are TAX-FREE.
14. Promo period is from January 01, 2008 to March 06, 2009.
15. All residents of Mangaldan are encouraged to participate in the RAFFLE DRAW.  Residents from other cities and towns are welcome to join the RAFFLE DRAW.
